Business Challenge

Kidney disease patients, such as those with chronic kidney disease (CKD) and end-stage renal disease (ESRD) typically manage 15-20 medications daily and have multiple comorbid conditions, which complicates their treatment. Since CKD symptoms are subtle, patients are often under-diagnosed for advanced CKD. Given that patients with kidney disease comprise less than 5% of SSM Health’s patient population but account for more than 20% of its total costs, the health system needed a solution that would predict the best time to provide patient treatments, improve outcomes and lower costs.

Analytics Solution

SSM Health, in collaboration Strive Health, developed the CareMultiplier analytics platform to predict adverse patient events. Using data from several datasets, more than 30 algorithms provide an overall risk score that informs clinicians of disease progression, along with the optimal timeframe for patient treatment. The predictive platform enables SSM Health clinicians to be more proactive with patient treatments and to bring a more focused approach to care.

Impact

SSM Health has significantly improved the quality of patient care and outcomes using the CareMultiplier analytics platform. For both CKD and ESRD populations, results indicate a reduction in acute utilization and a more than 25% reduction in emergency department utilization.
